First-generation antipsychotics, like Chlorpromazine, Fluphenazine, Haloperidol, and Perphenazine, can have more significant neurological side effects, but they are often cheaper, especially the generic versions. Some antipsychotics can be given as injections, which may be an option for those who prefer fewer pills or struggle with medication adherence.

Psychosocial interventions are also an important part of the treatment process, especially once psychosis recedes. These interventions may include individual therapy, social skills training, family therapy, vocational rehabilitation, and supported employment. The current landscape of schizophrenia treatment is primarily dominated by antipsychotics that block dopamine D2 receptors. However, these treatments do not fully address the broad range of symptoms that can present in schizophrenia, leaving a significant unmet need in this area.

Agents that act on muscarinic receptors, trace amine-associated receptor 1, serotonin receptors, and glutamate receptors are among those currently under investigation in phase 2 and 3 clinical trials. These novel mechanisms of action aim to address the full spectrum of schizophrenia symptoms more effectively and could represent a significant step forward in the management of the disorder.

Muscarinic receptor agonists and trace amine-associated receptor 1 agonists, in particular, have shown encouraging results in early clinical trials. These agents provide hope for meaningful improvements in the management of schizophrenia, offering potential new options for patients who do not respond adequately to current treatments.

While these emerging therapies are still under investigation, they represent the cutting edge of schizophrenia research and carry the potential to drastically improve the lives of patients with schizophrenia.
